Claims

We understand the hardships one has to undergo in case of loss of a loved one and that the bereavement caused is difficult to cope with. We, at Canara HSBC Oriental Bank of Commerce Life Insurance Co. Ltd, are dedicated to service our customers through simplicity and are guided to deliver our commitments on the happening of an unfortunate event.

Important Information:

1. All documents should be submitted in original or photocopies to be attested by any one of the following:
(1) An agent of the Company,(2) A Relationship Manager of the Company, (3) A Branch Manager of a distributing bank,
(4) A Bank Manager of a Nationalized bank with Rubber Stamp, (5) A Gazetted Officer, (6) A Head Master / Principal of
a Govt. School, (7) A Magistrate (8) Any employee of the company

2. In addition to the below listed documents, Canara HSBC Oriental Bank of Commerce Life Insurance
    Co. Ltd. reserves the right to call for any additional documents/ information

3. Submission of claim form/ any other documents should not be construed as acceptance of the claim

List of Claim Documents

Below is a detailed list of documents that you may be required to send along with the claim intimation form:

Death Claim

In the event of unfortunate death of the life insured, the following documents need to be submitted:

  • Death Certificate issued by Municipal authorities

  • Original policy document

  • Claimant Statement(available under download section)

  • Physician's Statement

  • Treating Hospital Certificate

  • Employer / School College Certificate

  • Hospital / Other treatment records

  • Photo Identification & address proof of the claimant

  • Post-mortem & chemical viscera report, if performed

For unnatural/accidental deaths, the following additional documents are required to be submitted:

  • Police reports (FIR, Panchnama, Police investigation report)

  • Newspaper cutting

  • Copy of Driving License of the insured in case of road accidents while insured was driving

Claim Process

The Claims department at Canara HSBC Oriental Bank of Commerce Life Insurance Co. Ltd. follows a simplified settlement process and endeavours to ensure that the commitments are delivered in a prompt and courteous manner.

The claim settlement process is as detailed below:

  • Intimation of the claim:

    The claim may be intimated by the nominee or close relative of the life insured to the company. The claim intimation must be in the form of a written communication letter / intimation form submitted to the respective bank branch or to the Claims unit at the head office(address under contact us heading).

    Also, the claim may be intimated over the telephone by calling us at the resolution centre number (1800-10-0003) or by writing an email to us at customerservice@canarahsbclife.in. This needs to be followed by a written intimation in order to register the claims.

    The claim intimation form is available at the respective bank branch and is also available for download from the website. It should be filled up by the claimant and submitted to the company.

    Note :

    In order to expedite processing of the claim, it is advisable that the intimation letter/ form must be submitted along with the proof of happening of the event i.e., the original or attested photocopy of death certificate and along with other documents as detailed under the list of documents for a death claim

School, (7) A Magistrate, (8) An employee of the company.

1. On receipt of the claim intimation, the company would send a claims pack to the claimant calling for the required
     documents/ clarifications to further process the claim.

2. The claimant must submit all the requirements to the respective bank branch / company's office address as mentioned in the
     claim letters / Claims unit at Head office at Gurgaon for further processing of the claim.
